FREQUENCY AND LENGTH
Supervision sessions are scheduled weekly or monthly for a 60 minute hour. If you show up late, you will not get credit for a full hour and will have to make this time up within another session. Master's Level LLPC's need to be supervised for a minimum of 100 hours for no less than two years. Doctoral Level LLPC's who have achieved 30 semester hours or 45 quarter hours need to be supervised for a minimum of 50 hours for no less than one year.

BILLING

A supervisee is expected to pay for supervision at the end of each session. I accept cash, personal checks, or credit card. If a supervisee arrives late to the supervision session, he/she may be billed for the entire 60 minute session, so please make every effort to be on time. I request a courtesy of a 24 hour notice in the event that you may need to cancel an appointment. Without a 24 hour notice, a supervisee will be charged a $30 cancellation fee unless the cancellation is an emergency (e.g. family death, car accident, illness). If a supervisee does not show for supervision, he/she will be charged a $30 cancellation fee. A supervisee will be billed $50 for any returned checks or insufficient funds. Supervision can not proceed until a supervisee's bill is fully paid, unless other arrangements have been made.

FEES
Fees for supervision are $115 per individual hour, based on income. Fees for group supervision may be reduced based on the number of supervisees attending. A maximum group of 4 will be $60 per hour session; a group of 3 will be $65 per hour session; and a group of 2 will be $70 per session.

SUPERVISION

FREQUENCY AND LENGTH OF COUNSELING
Counseling sessions are scheduled as needed by the client(s). I typically recommend weekly or bi-weekly counseling sessions so that progress remains consistent. Each individual, couple, or family session is 55 minutes long. Short-term or brief counseling will last around six to eight weeks, while long-term counseling may last a year or more based on the chronic nature of the illness or problem.

ATTENDANCE, PARTICIPATION, AND PROCESS
Attendance and participation in counseling is always voluntary. A client(s) progress is often based on his/her active participation. The more effort a client(s) puts into his/her counseling, the more he/she will get out of it. The first two sessions are needed for me to get to know you and your needs, to set up your goals, and discuss what you hope to get out of your counseling. The client(s) will be expected to fill out intake forms, HIPPA, and a contract form at the start of your first counseling session.

BILLING
Clients are expected to pay for counseling at the end of each session. I accept cash, personal checks, credit card, Health Savings Account cards, and Flexible Spending Account cards. If a client(s) arrives late to the counseling session, the client(s) may be billed for the entire session, so please make every effort to be on time. I request a courtesy of a 24 hour notice in the event that you may need to cancel an appointment. Without a 24 hour notice, the client(s) will be charged a $30 cancellation fee if the cancellation is not an emergency (e.g. family illness, car accident, illness). If a client(s) does not show for a scheduled counseling session, the client(s) will be charged a $30 cancellation fee. A client(s) will be billed $50 for any returned checks or insufficient funds. Counseling cannot proceed until a client(s) bill is fully paid, unless other arrangements have been made.

INSURANCE

Currently I only accept insurances that are out-of-network. Clients will be billed for services and can request a CPT code, a diagnosis, and the dates of counseling to submit to their insurance company to fulfill deductible requirements and/or to try to get reimbursement for counseling fees paid to this counselor.

FEES
Individual, couple, or family counseling fees are $115 per session. A client who requests doing telehealth or virtual sessions will be billed at $120 per session.

GROUPS

A group can be facilitated if there are 4 or 5 group members. Group counseling fees are $70 per 90 minute session per group member.

TERMINATION OF COUNSELING
A client(s) may terminate counseling at any point for any reason. If the client(s) are on probation or mandated to attend counseling, terminating counseling may result in other ramifications. I recommend that if a client(s) chooses to terminate counseling that we discuss other alternatives, such as setting up family/community supports, recommended books, spiritual consultation, or crisis intervention services. If a client(s) want a referral to another counselor/therapist, psychologist, or a psychiatrist, I will be happy to provide referrals.
